Many of your provider selection decisions will be based on geographical location, provider specialties and, in most cases, selecting a provider from the networks below will reduce your out-of-pocket expenses.
For 2005 new business, with certificate effective dates 1/1/05, and beyond, the primary PPO Network is Hygeia. If your policy is a renewal and the certificate renewal date is 1/1/05 and after, your PPO is also Hygeia. In most cases the network as identified with the PPO logo on your ID card is the Participating Network. However, if you have received an ID card, with a MultiPlan PPO logo, and you have a policy which is described above as being part of the Hygeia network, please contact MultiNational Underwriters office to request a new card.
